Updating the app fixes most issues. If you are a new member it may take some time to be accepted into the union and allowed to register for the app.

Transfers:

When transferring out of Utah or to another state, please contact the Teamsters Local in the area you are transferring to and contact Teamsters Local 222 to complete all other necessary tasks including enrolling in insurance and transferring membership.

How do I transfer within the state of Utah?
To transfer as a part-timer within the state of Utah you will need to fill out an Opportunity Request Form

How do I apply for a part-time employee transfer outside the state of Utah?
If you are a part-time employee who wishes to transfer to another location for non-educational purposes, you may submit a written request, as long as the transfer location is more than fifty (50) miles away from your current location. Search “Requesting a transfer – part-time union employees” in UPSers for more information.

How do I apply for a part-time educational transfer outside the state of Utah?
You will need to meet the minimum requirements, provide proof of your college enrollment and class schedule and fill out and submit the transfer request form. Search “Educational transfer process for part-time union employees” in UPSers for more information.

Insurance related questions: 

How do I enroll in insurance?
UPS employees are eligible for insurance after 9 months of work at UPS. You must fill out the enrollment form and send it to Southwest Service Administrators via fax or mail. It can take up to 45 days to process or update your enrollment form and for your medical cards to be sent.

How do I add or take off my spouse or child from my insurance?
You must fill out the enrollment form and send it to Southwest Service Administrators via fax or mail. 

If adding a spouse, you will need to include a marriage certification. If adding children, will need to include birth certificates. It can take up to 45 days to process or update your enrollment form and for your medical cards to be sent.

What are the eligibility requirements for insurance coverage?
Part-time employees must punch in once a week for coverage for that week. 

Full-time employees must punch in once in a calendar month to receive health insurance for a month period. Full-time employees insurance is forwarded 2 months, this means when you punch in in January it is covering your insurance for March. Please call Southwest Service Administrators for the most accurate, up to date information regarding your coverage.855-292-7954

Medical Leave

I need a medical leave, where do I start?
If you were injured at work you must report the injury to UPS and follow their process. 

If you were injured off the job or have a qualifying medical event and will require more than 3 consecutive days off then apply for Short Term Disability.

If you need intermittent days off due to a serious medical illness, to take care of an immediate family member with a serious medical illness or for the birth of a child, adoption or foster child apply to FMLA.

Do I keep my insurance while on an approved medical leave?
For workers compensation, you will continue to have coverage for up to one year.
For FMLA, you will have no lapse in coverage.
For short term disability, you will continue to have coverage for 6 months.
For long term disability, you will continue to have coverage for 6 months.

Do I need to apply for a Withdrawal Card with Teamsters Local 222 when taking a medical leave?

You must notify Teamsters Local 222 when you return to work to be taken off of Withdrawal. Call (801) 972-1898 and speak with Amanda (ext. 20) or Carly (ext. 21).

Q: What is a withdrawal card and why is it so important?
A: If a member does not work any days in the bargaining unit during the current calendar month the member shall be issued an honorable withdrawal card upon request. All financial obligations including dues must be paid at the time of the request. It is very important to request a withdrawal card in a timely manner as dues can accrue until a withdrawal card is requested. Upon returning to bargaining unit work, you are required to contact the Local Union immediately to deposit your withdrawal card and return to “active” member status.

Q: What happens if I don’t request an honorable withdrawal card?
A: If a member does not request a withdrawal card, he/she shall stand suspended at the end of the third month, and shall not be entitled to any rights or privileges of membership. Upon returning to employment at the craft a re-initiation fee plus back dues will be required for entitlement to the rights and privileges of membership. 

Short Term Disability and Long Term Disability

Short term disability is for when an employee is injured off the job or has a qualifying medical event and will require more than 3 consecutive days off then apply for Short Term Disability.

Short term disability can be taken for up to 6 months. If more time is required then you must apply for long term disability which lasts an additional 6 months. Only full-time UPS employees are eligible for long term disability.

You must notify The Hartford of your leave of absence, in addition to sending the application to Southwest Service Administrators 866-825-0186

How do I get my employer to fill out section B of the form?

How do I check the status of my short term disability claim?

How much will I be paid while on short term disability?
Employees will be paid by Southwest Service Administrators, contact SSA for issues regarding payment while on short term disability.

For part-timers: 66% of your wage up to $500/week
For full-timers: $300/week

Do I keep my insurance while on short term disability?
Yes.

Are part-timers eligible for long term disability?
Only full-time UPS employees are eligible for long term disability.

What do I do when I am ready to return to work?
When you are ready to return to work, you MUST call your management team and The Hartford at 866-825-0186 to report your return to work. This is an important step to ensure that you do not experience issues with benefits and/or pay. The Hartford does not require a letter authorizing your return.

On the Job Injuries and Workers Compensation

If you were injured at work you must report the injury to UPS and follow their process.

What is TAW or light duty?
TAW is Temporary Alternative Work or commonly known as light duty. “This program is designed to provide temporary work opportunity to those employees who are unable to perform their normal work assignments due to an on-the-job injury.” 

For on the job injuries, the company may allow you to work for 30 days and accommodate your restrictions before placing you in workers comp. 

While on TAW, you are entitled to your daily guarantee with a start time no more than 2 hours earlier or 2 hours later than your normal start time, provided the work is available.

How long do I keep my insurance while on workers comp?
For up to one year after your workers compensation claim began.

FMLA (Family Medical Leave Act)

Am I eligible for FMLA?
Full-timers must have worked 1,250 hours in the past rolling 12 months to be eligible.
Part-timers must have 3 years with the company and have worked 625 hours in the past rolling 12 months. 

What are the qualifying reasons for FMLA?

  • 1. Birth of a child;
  • 2. Adoption or placement for foster care;
  • 3. To care for a spouse, child, or parent of the employee due to a 
  • serious health condition;
  • 4. A serious health condition of the employee.

How do I get on FMLA?
Call The Hartford 866-825-0186 and follow the process.

Is FMLA paid?
No, FMLA is unpaid leave.

Do I keep my insurance while on FMLA?
Yes.

Can I take a quarter or half day for FMLA?
Only if your FMLA documentation specifies allowing it.Can my employer burn my entitlements while I use FMLA?
Employees are allowed to retain up to two weeks of accrued paid vacation, the remaining entitlements may be cashed out by the employer when the employee uses FMLA.
